We want to manage your appointment effectively, and we value your time and safety. You can help by printing and completing the relevant patient forms before your arrival.
All patients are asked to fill out these forms prior to their exam, even patients who have had previous exams at New York Private Medical Imaging. Although we keep all your prior information and refer to it, we note any changes in information, as this is critical for the interpretation of your exam. If you prefer, you may complete the forms when you arrive for your appointment.
Please note: You are not required to fill out these forms for the following exams - Digital X-ray, Digital Fluoroscopy, Sonography/Ultrasound.
