In-Person CD Requests

Patients may request a CD copy of their imaging studies by visiting any Radiology Regional imaging center or Medical Records office.

No form is required. CDs are created while you are on-site. Find a Location near you.

The Medical Records office is located at: 3660 Broadway, Fort Myers, FL 33901 
Please use the parking lot in front of the office building and proceed to the main door.
